Eligibility: Open to legal residents of California who are patients of Sponsor. Sweepstakes is open to all ages; however, entrants under the age of 18 must have the consent of a parent or legal guardian to participate. Void where prohibited. Employees of the Sponsor, its affiliates, subsidiaries, advertising and promotion agencies, and their immediate family members are not eligible.
Promotion Period: Begins September 1, 2025 at 12:00 AM PT and ends September 30, 2025 at 11:59 PM PT.
How to Enter:

Appointment Entry: Schedule and attend a dental appointment at Sponsor’s office during the Promotion Period — one entry per patient.

Alternate Method of Entry: Hand-print your full name, mailing address, phone number, and email address on a 3” x 5” card. Prize: One (1) Nintendo Switch 2 System. Approximate Retail Value (ARV): $449.99. Prize is non-transferable, no substitution or cash equivalent permitted.
Winner Selection & Notification: Winner will be selected in a random drawing from all eligible entries on or about October 4, 2025. Odds of winning depend on the total number of eligible entries received. Winner will be notified by phone and/or email. Winner must respond within 7 days of notification or prize may be forfeited and awarded to an alternate winner.
Publicity Release: By accepting the prize, the winner agrees to allow Sponsor to use their name, likeness, and city of residence for promotional purposes without additional compensation, except where prohibited by law.
General Conditions: Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes if fraud or technical failure impairs the integrity of the Sweepstakes. All decisions by Sponsor are final.
